Definition

Necrosis of bowel wall 2/2 microbial invasion

Pathophysiology

  1. Usually involves the CECUM
  2. Typically occurs 10-14d after cytotoxic therapy

Diagnosis

  1. Fever
  2. RLQ pain
  3. Nausea
  4. Vomiting
  5. (mimics appy)

Workup

CT A/P: cecal distention and wall thickening

Treatment

  1. Bowel rest
  2. NG suction
  3. IVF
  4. Broad spec abx

Disposition

Surgical consultation (right hemicolectomy)

Prognosis

Mortality 50%
