440–500 ms Borderline/Prolonged — Monitor closely. Review medications for QT-prolonging drugs.
>500 ms Significantly prolonged — High risk for Torsades de Pointes. Discontinue offending agents. Check Mg²⁺/K⁺/Ca²⁺.
