| 440–500 ms | Borderline/Prolonged — Monitor closely. Review medications for QT-prolonging drugs. |
| >500 ms | Significantly prolonged — High risk for Torsades de Pointes. Discontinue offending agents. Check Mg²⁺/K⁺/Ca²⁺. |
