ACS Anatomical Correlation Chart

ST Elevation Location Coronary Artery
V1-V2 Septal Septal branch
V2-V4 Anterior LAD
I, aVL, V5, V6 Lateral Circumflex
I, aVL, V2-6 Anterolateral LAD + circumflex = Left main or 2 critical lesions
II, III, aVF

aVL most common lead to see reciprocal change

Inferior RCA

V1>V2
III > II
STD in V2 & not in V1
STE common in V1, but may see isoelectric

Right ventricle RCA

ST Depression in V1, V2, V3; R>S in V1

Posterior aka Inferolateral RCA (90%), LCA (10%)
avR>V1

Doesn't apply in SVT

Anterolateral Left Main
