Background

Most common in children aged 4-7 years

Diagnosis

CENTOR CRITERIA

  1. History of fever
  2. Absence of cough
  3. LAD
  4. Tonsillar exudate

DDx

  1. viral pharyngitis
  2. PTA
  3. retropharyngeal abscess
  4. GC pharyngitis

Treatment

CDC Guidelines for adults >15yrs old

POINTS TREATMENT

4 Rx presumptive

3 Rx Presumptive

2 Rapid Strep Test

1 Do not test/treat

0 Do not test/treat

Treatment = PCN (erythro if allergic)

3 criteria = 40% PPV

4 criteria = 50% PPV

No need for cultures after neg rapid test
