  • Starts with the Social Work Consult (goes to their discharge planner - Medical Case Worker 6a-11p)
    • Homeless Shelter - need to be out the door by 2pm
    • Domestic Violence Shelter
    • SNF
    • Recuperative Care (housing for health ends 12/31/2025)
    • Assisted living
    • Home Health (need a home)
    • Respite Care
    • Board & Care (need funding)
    • Rooms for rent
    • SW Office will give TAP card for bus fare (otherwise property room for after-hours, although SW office open by the time busses are running in the AM; OCN can get taxi voucher; no taxi)
    • Sobering facilities (how does the intoxicated patient safely get there)
