Administration

  • Type: Atypical antipsychotic
  • Dosage Forms:
  • Routes of Administration: PO, IM
  • Common Trade Names: Geodon

Adult Dosing

  • Mania/psychosis: 40 to 80 mg PO BID
  • Acute agitation: 10-20mg IM q2-4h, 40mg max daily

Pediatric Dosing

  • Age 10-17: slowly titrate up to target dose 20-80mg BID

Adverse Reactions

Serious

  • Prolonged QT, torsades de pointes, syncope
  • Neuroleptic malignant syndrome, tardive dyskinesia, seizure
  • Bone marrow depression
  • Dysphagia
  • Hyperglycemia, diabetes
  • Priapism

Common

  • Extrapyramidal disease, dystonia, tremor, akathisia
  • Dizziness, asthenia, somnolence, anxiety
  • Nausea/vomiting, diarrhea, constipation
  • Weight gain
  • Rash
  • Visual changes
  • URI

Pharmacology

  • Half-life: 2-7h
  • Metabolism: Hepatic: CYP3A4, and CYP1A2
  • Excretion: Mostly fecal

Mechanism of Action

  • D2 and 5HT2 antagonist. Also strong α-1 blocker, antihistamine-1.
