Administration

Adult Dosing
- 1.25-5mg IV q6h
- 0.625mg IV initially, with repeated dose of 0.625mg if inadequate response for the following patients:
- CrCl <30
- Severe volume or salt depletion, recent HD, or concomitant diuretic use
- CHF
