Adult Dosing
- 25-300mg PO daily
Pediatric Dosing
- 0.5-0.75 mg/kg/day OR 20-25 mg/m(2)/day PO in 3 doses
- 0.25-0.35 mg/kg/day OR 12.5 mg/m(2)/day IM

- Pregnancy Rating: D
- Lactation risk: Infant risk cannot be ruled out
- Renal dosing: no adjustment
- Hepatic dosing: no adjustment
Contraindications
- Allergy to class/drug
- Systemic fungal infections (except when used for replacement therapy in adrenal insufficiency)
- Concomitant administration of live or live attenuated vaccines
