Adult Dosing
- Atrial fibrillation, cardioversion ("pill-in-a-pocket")
- 200-300mg PO once
- Paroxysmal atrial tachycardia (prophylaxis)
- 50-150mg PO BID
Pediatric Dosing
- 25-100 mg/m(2) PO 2-3 times per day, MAX dose 200 mg/m(2)/day
Special Populations
- Pregnancy Rating: C
- Lactation risk: Infant risk minimal
- Renal dosing: adjustment may be required
- Hepatic dosing: adjustment may be required
Contraindications
- Allergy to class/drug
- Cardiogenic shock
- Right bundle branch block (without pacemaker)
- 2nd or 3rd degree AV block (without pacemaker)
Adverse Reactions
Serious
- Heart block, sinus node dysfunction
- Torsades de pointes, ventricular tachycardia, ventricular fibrillation
- Cardiac shock, cardiac arrest
- New onset or worsening heart failure
Common
- Dizziness, fatigue, blurred vision
- Nausea
- Dyspnea
