Adult Dosing

Dysuria

  • 100-200mg PO TID x 2 days

Pediatric Dosing

Dysuria

  • 6-12yo: 4mg/kg PO TID x 2 days
  • >12yo: 100-200mg PO TID x 2 days

Special Populations

  • Pregnancy Rating: B, contraindicated in pyelonephritis during pregnancy
  • Lactation risk: safety unknown, inadequate literature available; caution advised

Renal Dosing

  • Adult:
    • CrCl 50-80: give q12-16h
    • CrCl<50: contraindicated
  • Pediatric:
    • CrCl 50-80: give q12-16h
    • CrCl<50: contraindicated

Hepatic Dosing

  • Adult:
    • severe hepatitis: contraindicated
  • Pediatric:
    • severe hepatitis: contraindicated

  • uremia
  • pyelonephritis during pregnancy
  • renal impairment
