Retrobulbar hemorrhage

Background

  • Most often due to blunt orbital trauma
  • Can cause loss of vision due to abrupt increase in intraocular pressure
  • Also known as retrobulbar hematoma

Clinical Features

  1. Pain
  2. Proptosis
  3. Decreasing vision

Diagnosis

  1. Tonometry
    1. Only measure once globe rupture has been ruled-out
  2. CT Orbit

Periorbital swelling

Proptosis

No proptosis

Treatment

  1. Emergent ophto consultation
  2. Consider lateral Canthotomy if IOP >40
